- In a modern world where advanced Biosyn implants mean everyone is subjected to a relentless visual stream of information they call the 'Mind's Eye', privacy and anonymity as we know it has vanished. With everyone's personal information on public display and each person's life recorded down to the millisecond, personal information is downloaded to a vast grid called 'The Ether', a database which law enforcement can access and use to prosecute criminals.
Sal Frieland (Clive Owen) is faced with a series of shocking murders which appear to be linked. Unable to identify the killers it soon becomes apparent that what they have in common signals a major breakdown in the system on which society has come to rely. Detective Charles Gattis (Colm Feore) is Sal's boss and calls him on a homicide case. The peculiarity is that the killer has hacked into the victim's mind's eye. Hence the last few minutes of the victim's life show victim's vision from the killer's eye. The killer shoots the victim, and the victim himself could not see anything while his Mind's eye was hacked. The weird thing is that from the data available it looks like the victim was alone at the time of the shooting.
As he delves into the case, Frieland encounters a woman (only known as the girl (Amanda Seyfried) seemingly with no identity. Sal sends a copy of his minds recording to Charles, but later finds that his own file of the recording has been deleted. Looks like someone hacked into the live recordings of both Sal & Charles and deleted the copy of "The Girl's" recording.
It is implied that Sal used to have a son with his ex-wife Kristen (Sonya Walger), from who he is now divorced. Something he initially assumes is a minor glitch becomes the first clue that security of 'The Mind's Eye' has been compromised, sending Frieland off on a mission to find a someone who effectively doesn't exist before they kill again. Sal intercepts the double murder of lesbians, which employed the same tactics. He responds and finds the killer still inside the mansion. He gives chase, but the killer is able to escape after hacking Sal's own Mind's Eye.
Sal figures that the victims have hired the hacker to erase some part of their digital footprint, and when the hacker collects their money, he/she kills the customers to erase any trace of their own. Sal agrees to be the bait to lure the hacker. Sal creates a digital profile of an engaged stockbroker, who hires a hooker for a night and now wants to erase that so that his fiance doesn't come to know about it. He posts a message on a bulletin board used by the Hacker's previous victims. The Girl takes the bait and fixes a meeting with Sal. Unknown to her Sal's colleague Lester (Joe Pingue) is nearby and hacks into the Girl while she hacks into Sal to erase his files. Lester tries to find The Girl's true identity, but in unable. The Girl leaves.
Meanwhile Charles gets a tech specialist on his team Cyrus Frear (Mark O'Brien). Cyrus completes Lester's trace and finds the Girl's live records. The hacker discovers the trace and severs all links. Cyrus is not sure if the girl saw Sal or Lester or Cyrus himself. Commissioner Joseph Kenik (Iddo Goldberg) steps in to refocus the team. To him it's not a homicide, but a precursor to an undetectable crime wave. Sal goes undercover again and has sex with the Girl. Surprisingly the girl doesn't shoot Sal, bucking her own trends so far during the investigation. After sex, the Girl discovers that Sal's memories are implanted and figures out that he is a cop. Instead of shooting Sal, Sal finds that Lester has been shot who is in the adjoining room running a trace on The Girl.
The Girl then connects into Sal's live feed and superimposes all sorts of horrors into his vision. Including the sight of his son dying in a car crash when Sal was supposed to looking after him but took his eyes off him for a second. He also finds the files of all his happy memories deleted. Sal cannot trust anything he sees anymore. She even frames Sal for a murder, which leads to him being suspended off the police force.
Sal tracks down the Girl's apartment and pays a visit. The Girl argues with Sal that she never shot anyone, but someone made it look like that she did by manipulating live records. Sal believes her and lets her go. Charles is upset with Sal for this.
The Girl distracts the police watching Sal's apartment and goes in. At that moment the real hacker shoots at Sal & attacks The Girl. Cyrus is revealed to be the real hacker and serial killer. While The Girl stalls Cyrus, Sal recovers and shoots him dead. It emerges that Cyrus wasn't even on the police force. He had forged his entire entity. Sal meets the Girl one more time and she refuses to divulge her identity to him.
