- The story of a group of young friends and the mysterious events that occur in the rural village of Hinamizawa.
- In this continuation of the hit horror anime, secrets are revealed as Rika Furude desperately attempts to save Hinamizawa and her friends from their tragic fate
- Early summer, Keiichi had just moved to a remote mountain village. As he becomes close friends with a group of girls, he was enjoying his life in the village. However when he starts to become suspicious that Rena, Mion and the others that Keiichi trusted may be deeply involved in successive murders that occur every summer, the situation around Keiichi gradually starts to turn. As if the girls' characters had completely changed, a sewing needle is found in his food, he is chased by mysterious men, and is involved in an accident which could have killed him. He could no longer trust anyone, and then one mystery leads to another, bringing out a shockingly bloody ending.
- With an outbreak going on and Neptune missing, it's up for Nepgear and the other CPU Candidates, who have been reawakened after an incident caused them to be put into cryostasis for two years, to stop the outbreak and find Neptune.
- Keiichi hears the odd circumstances surrounding Satoko's older brother, Satoshi. It seems he simply left the village, but when Keiichi mentions Satoshi to Shion and Rena, each reacts in an unusual manner.

- At school, Rena's attitude brings smiles to everyone around her - but things are different at home. Her mother left the family for another man, and she worries her father's new girlfriend is only after his money.
- When he realizes he forgot to pack his bathing suit while rushing to meet his friends at the pool, Keiichi borrows a pair of trunks from Mion's uncle, who claims that these trunks have the magical ability to make women attracted to whoever writes their name on them and wears them for three hours. Mion hears from her uncle that he had made a mistake in describing the abilities of the trunks, and that the person whose name is written on the briefs will fall in love with the wearer, meaning Keiichi, who had written his own name on them, will fall in love with himself. The girls try to talk him into taking the trunks off. Keiichi thinks it's a trick so they can force him to wear an embarrassing pair of trunks they provided. The girls rally everyone at the pool to chase him down and retrieve the trunks by force, even going as far as turning Keiichi's only supporters (Tomitake, Oishi, and Irie) against each other to fight over wearing the trunks for themselves. Finally, the trunks are removed before time runs out. The naked Keiichi still refuses to wear the girls' embarrassing trunks.
- Keiichi, Tomitake, Irie and Oishi speculate over punishment games for the girls after Keiichi's recent string of losses.
