Following the withdrawal of Pierce Brosnan, O'Loughlin was a candidate for the role of James Bond, until the role was recast with Daniel Craig in 2005, in preparation for Casino Royale (2006).

Graduated from Australia's National Institute of Dramatic Art in Sydney in June 2002, after completing a full-time three year Bachelor of Dramatic Art program.

In early March 2012, CBS announced that O'Loughlin would miss shooting some episodes of Hawaii Five-0 (2010) while reportedly seeking rehab treatment for dependency on, allegedly, pain medication which had supposedly been prescribed to him for a reported shoulder injury. For several episodes, he had only cameo appearances, which were explained as McGarrett being on a covert mission to North Korea.

In August 2013, Los Angeles, California based charity, "Australians in Film" selected him to be honored with their "Breakthrough Award" at a special ceremony to be held in October 2013 in Los Angeles.
