- Christian is a little annoyed. He doesn't understand why Tanja is looking for her own apartment in Eisenach and doesn't want to move into his villa. Is she not taking the relationship as seriously as he is, or is his friend Michael right when he advises Christian not to be so clingy right from the start? In practice, the two are developing into a true dream team. Whether it's Tanja's case of a young pregnant woman with complications or Christian's tricky diagnosis of an elderly patient whose nonspecific symptoms are extremely difficult to interpret - the two doctors complement each other excellently, and Tanja's wit and charm are very popular with the patients and the entire team. At the same time, Timo receives an "immoral" offer from Professor Bender to move to the hospital as head of radiology. Timo hesitates. Will he let Christian and the practice down?
- A patient is causing Christian great concern: Georg Knapp, a widowed pensioner, is receiving inadequate care from the nursing staff at his retirement home and soon has to be hospitalized due to an infection. Lisa's husband, Michael, now chief physician at the local hospital, is only providing routine care - with life-threatening consequences for the patient. At the practice, Christian clashes with Lisa because she wants to offer customized health services as an additional service. Christian isn't at all in favor of this "money-making" scheme. But the decision must be made jointly, as Christian is no longer the sole boss. Due to the Kleists' financial difficulties, Christian is forced to ask Michael for additional income as an emergency doctor at the hospital. Anna's financial situation is also becoming increasingly difficult, so she seeks legal representation at the district court with the help of a former mentor. Her first assault case presents her with a moral dilemma. Nora, on the other hand, has every reason to be happy, as Lisa hires another medical assistant for the practice to provide relief. Meanwhile, the erotic tension between Lisa and Timo grows ever stronger and ultimately erupts in an affair. Lisa regrets this shortly afterward, but she can't bring herself to tell Michael the truth. Anke is also plagued by a guilty conscience: While Piwi passionately clings to the idea of Singapore, Anke is becoming less and less enthusiastic. When Piwi learns that she's seeking advice from Christian, of all people, a heated argument ensues. Piwi disappears without a trace. Clara, Sabrina, and Lauri introduce their band "Pink Slip" to a music producer in Berlin. But Clara's enthusiasm is severely dampened when the producer suggests she kick Lauri out of the band. Now Clara must decide what's more important to her - her dream of a band career or her best friend.
- Christian's regular assignments as an emergency doctor are now pushing him to the limits of his endurance. To be able to meet the needs of his patients during the day, he continues to take tranquilizers. An elderly patient, Elfriede Fischer, presents to the practice with acute heart problems. She urgently needs a new heart valve, but she is terrified of the procedure. Since transfer to an outside clinic is not an option, Michael suggests a modern, gentle surgical procedure in Eisenach, which leads to a clash with Professor Bender. Bender insists on a traditional, less expensive procedure for economic reasons. When Michael nevertheless decides in the patient's best interests, thereby overriding his boss, he is threatened with a warning. At the same time, Christian and his colleagues Lisa and Timo are investigating a mysterious case: The young gymnast Melanie is about to compete in an important competition and is complaining of chronic exhaustion. However, her values are within the normal range. Christian finally discovers a possible cause for Melanie's symptoms, which is much more serious than suspected. Piwi has still disappeared without a trace after the argument with Christian. Anke and the Kleists wait in vain for a message from him and gradually begin to worry. Lisa manages to find Piwi and get through to him. When Piwi tries to talk to Christian, he reacts surprisingly harshly: He's fed up with his son's antics and insists on a decision that will affect the entire practice. Anna struggles with her dubious mandate. She seeks advice from Christian. His medical analysis of the case makes her seriously doubt her client's innocence. Meanwhile, Lisa tries to avoid Timo. But when he reveals his sensitive and thoughtful side for the first time, she feels an unusual closeness to him and has to make an effort not to succumb to his advances again.
- Lisa Kleist's pregnant patient, Madita Krämer, is desperate: Her husband, Thomas, is suffering from uncontrollable sleep attacks. His sudden sleeplessness is putting the heavily pregnant woman under great strain and even putting their son, Max, in grave danger. Christian Kleist can't find anything at first, but with the help of his daughter Lisa and radiologist Timotheus von Hatzfeld, he uncovers Thomas's mysterious illness. Christian's daughter, Lisa, is getting along surprisingly well with Hatzfeld's new colleague. His advances soon impress her more than she'd like, and tension begins to build between the two. The atmosphere between Christian and Piwi is becoming increasingly icy, as Piwi is determined to give up his responsibilities at the practice so he can accompany Anke to Singapore. He doesn't want to live solely for work like his father: Anke and his private life should take priority. While Christian's youngest son, Paul, is having trouble at school, his partner, Anna, is struggling to secure new clients after her main client goes bankrupt. Fortunately, she's able to mitigate a costly warning for Paul's illegal downloads, but the Kleists' financial situation remains difficult. After Piwi's accusations and Paul's problems at school, Christian increasingly doubts his abilities as a father. But work stress and money worries soon take their toll on him so much that he secretly resorts to tranquilizers. Meanwhile, Clara has a clever idea to actively help her family out of their financial predicament.
- Margarete Kuske comes to the practice with mysterious breathing and heart problems. Neither Christian's nor Timo's examinations provide any concrete answers. Soon, she becomes an acute emergency in the practice, forcing the medical team to perform a risky procedure. Christian's nightly shifts as an emergency doctor increasingly push him to his physical limits. Nora notices the missing medications, which leaves Christian at a loss for words. To avoid giving himself away, Christian finally asks Michael for help. Meanwhile, Michael is officially reprimanded for going it alone during Mrs. Fischer's heart valve surgery. But does it make sense to take legal action against his boss, Professor Bender? A discussion ensues between Piwi and Anke, during which he apologizes for his immature behavior. Anke is willing to forgive Piwi, provided he finally takes responsibility for their relationship and for the Kleist practice. She wants to start her trainee position in Singapore alone. In the joint vote for Piwi's future in the practice, Lisa and Timo vote in favor, but Christian is still unconvinced. The mood in Clara's band, "Pink Slip," is tense: Clara and Sabrina are still struggling with the fact that they turned down the Berlin music producer's offer so that their friend Lauri could remain in the band. Lauri herself realizes that she's not the best guitarist. An unexpected family reunion: Inge and Bernd suddenly appear at the villa's door. They have unexpectedly cut short their world tour. Christian pricks up his ears when Inge reports she has pneumonia.
