Earlier in the movie, the marriage counselor had asked how many times (presumably per week?) the Smiths engaged in sexual activity. Jane's response was that she didn't "understand the question" and John's response was "Does that include weekends?".
The takeaway here is that these two seemingly-ridiculously-attractive assassins just aren't sleeping with one another regularly, if much at all, when they first go for counseling but John's finger-splayed response "Ten." to the "sex question" seems to indicate they've since begun cultivating quite a robust sexual element to their marriage.

Married for five (or six) years, John (Brad Pitt) and Jane (Angelina Jolie) Smith seek marriage counseling in order to get their quiet, mundane marriage back on track. In flashback, they tell the therapist how they first met in Bogotá, Colombia, married shortly afterwards, and set up housekeeping together, living a normal life in a normal suburb, and working normal jobs until they learn that they are both really assassins hired by competing firms and that their next targets are... each other.
The film is based on a script written by English-born American screenwriter Simon Kinberg, who came up with the idea after listening to several friends describing marriage therapy they were receiving.
By the way the plot of the movie is very similar to the "While Black Moon" novel was written by Russian writer Alexandre Bondar that was published at 2003.
The unrated version of Mr. & Mrs. Smith not only offers new story elements but action scenes as well. On the other hand, several scenes have been removed from the Director's Cut.
