- A politically-minded enforcer's misguided trust in his lieutenant leads him to suspect his wife of infidelity in this adaptation of Shakespeare's 'Othello'.
- Advocate Raghunath Mishra has arranged the marriage of his daughter, Dolly, with Rajan, knowing fully well that Dolly loves Omkara Shukla. Before the marriage could take place, the groom's party is attacked, they flee, and Dolly is reportedly abducted. Raghunath is able to trace Dolly to Omkara, a criminal and hit-man, a verbal confrontation ensues until Politician Bhaisaab telephonically intervenes, and a crestfallen Raghunath faces the reality that Dolly was not abducted but is here with Omkara by her own free will. He warns Omkara, and departs. Shortly thereafter, Bhaisaab is shot at and wounded, announces that Omkara should stand in the next election, and as a result, Omkara appoints one of his lieutenants', Keshav Upadhyay in his place as the 'Bahubali". Omkara realizes that he may have blundered in having Keshav succeed him, as Keshav is unable to control his temper when under the influence of alcohol, which puts him in the bad books of Omkara. Then Omkara suspects that Keshav is having an affair with Dolly, as both had been fellow-collegians. With Omkara's marriage with Dolly getting close, he asks his other lieutenant, Ishwar Tyagi, to obtain proof of Keshav's affair with Dolly or face the consequences, as Omkara remembers Raghunath's warning that when a daughter is not true to her father, she will never be true to anyone. - Omkara "Omi" Shukla (Ajay Devgn) is an enforcer for a gang that commits political crimes for the local politician Tiwari Bhaisaab (Naseeruddin Shah). Ishwar "Langda" Tyagi (Saif Ali Khan) and Keshav "Kesu Firangi" Upadhyay (Vivek Oberoi) are his closest lieutenants. Langda gatecrashes a wedding procession and challenges Rajju (Deepak Dobriyal), the groom, to try and stop Omkara from abducting the bride, Dolly Mishra (Kareena Kapoor). Rajju fails and the wedding never takes place.
Omkara is a hardened goon, equivalent to a capo, a man with principles who sees life in extremes, either as good or evil. He is the illegitimate child of a higher-caste Brahmin man and a lower-caste woman. Dolly is a playful, innocent, young woman, smitten by Omi. She is the one who expresses her love for him and lays the basis of their relationship. Throughout the narrative, other characters are shown to doubt her innocence, including her father. Omi finds her personality ambiguous, resulting in the tragic end.
Dolly's father, advocate Raghunath Mishra (Kamal Tiwari), is enraged with Omkara and at one point threatens to kill him. To sort out the misunderstanding, Bhaisaab arranges for Dolly to appear before her father; she clarifies that she eloped with Omkara and was not abducted. Her father remains disgusted that she is in love with a man from a lower caste and leaves the village in shame. He tells Omkara that a girl who couldn't be loyal to her own father, will never be loyal to anyone else.
Omkara eliminates a powerful electoral rival by exploiting an MMS sex scandal. He also eliminates the rival's entire local enforcing team, when they insult Omkara's relationship with Dolly. Bhaisaab is elected for parliament and Omkara is promoted to the candidate for the upcoming state elections. Omkara appoints Kesu over Langda as his successor once he enters politics himself, as he feels Kesu, who hails from the upper classes and is college-educated, stands a better chance of winning over the younger, educated voting crowd than Langda, who is rustic and dropped out of school.
Langda (which means limp in Hindi) is the catalyst in the story. He had been a loyal right-hand man to Omi for years and expected to be the next Bahubali. Kesu's promotion gives a crushing blow to his ambitions and brings out the evil in him. His jealousy and hatred towards his kin is further encouraged by chiding remarks from Rajju.
Kesu is Omi's other deputy. His epithet comes from his knowledge of English. He is college-educated and urban, known to be somewhat of a Casanova. He is important to Omi for his political contacts in the students. After admitting to having "known" a string of women, he falls in love with Billo and proposes marriage. He is shown to be impatient and easily frustrated with a low threshold for alcohol. Rijju takes advantage of this rift and flames the feelings of revenge inside Landga. Rijju wants to use Langda to satisfy his own hunger for revenge against Omkara for stealing Dolly from her.
Langda, disappointed with Omkara's poor judgment and jealous of Kesu, his younger, less-experienced superior, decides to take revenge. He first causes a violent brawl between Kesu and Rajju (Rajju smoke a cigarette in front of Billo, which made her cough) by taking advantage of Kesu's low threshold for alcohol (Kesu didn't want to drink, but Langda egged him on publicly by making fun of him in front of the drunk troops), which damages Omkara's faith in Kesu.
Langda has Kesu convince Dolly to mollify Omkara (to forgive Kesu), but at the same time uses Kesu's visits to Dolly (in which Kesu was teaching Dolly a guitar song that Dolly wanted to sing as a surprise for Omkara) as evidence to suggest to Omkara that the two of them are having an affair. Langda additionally has Indu (Konkona Sen Sharma), his wife and Omkara's sister, steal an expensive piece of jewelry that Omkara gifted to Dolly (which was Omkara's traditional family heirloom), and encourages Kesu to gift it to his girlfriend Billo Chamanbahar (Bipasha Basu). When Dolly asks Omkara to forgive Kesu, it puzzles Omkara further. Langda asks Omkara that since Kesu and Dolly studied at the same college, it is likely that Kesu perhaps dated Dolly at some point.
Billo is later used to lure Kichlu (Who tried to assassinate Bhaisaab), a rival gang enforcer, out of hiding, so Omkara's gang can assassinate him. Omkara arrives at the ambush and spots Billo wearing the missing jewelry; he murders Kichlu and his gang in a rage. Langda sees Billo dancing with the jewelry on her. He uses that to spin a tale in front of Omkara about how Kesu used an expensive piece of jewelry to seduce Billo for sex. Omkara asks Dolly to produce the jewelry and she can't.
Omkara forces Langda to unambiguously tell him whether Kesu and Dolly are having an affair. Langda insists that they are, making a number of additional false claims to support his story. Omkara wants decisive proof. Langda steals Kesu's phone and gets Rijju to tell Billo that Kesu is spending time with another girl, when she calls Kesu's phone. Rijju then takes Billo to meet Kesu and calls Kesu to say that Billo is with Langda & is angry.
Meanwhile Langda was with Omkara who was going got his wedding night with Dolly. Langda uses this call to convince Omkara that Kesu is referring to Dolly in the conversation (while Kesu was talking about Dolly). Finally Langda produces the physical jewel that Billo threw back at Kesu as irrefutable proof that Dolly gave it to Kesu.
Convinced of the affair, Omkara smothers Dolly to death on their wedding night. Elswhere, Langda attempts to have Kesu assassinated, but the bullet merely grazes his arm. Indu hears gunfire and rushes to Omkara's aid, where she finds Dolly's corpse and the recovered jewelry. She confesses to having stolen the jewelry for Langda, making Omkara realize that Langda is responsible for his fatal misunderstanding of the situation. Indu slashes Langda's throat in retribution, while Omkara commits suicide and dies next to his wife.
