The relationship of a contemporary married couple, charting their evolution over a span of years by cross-cutting between time periods.
The film's title comes from Tom Waits's album of the same name.
I'm gonna hit you in five seconds. I'm gonna hit you in five seconds if you don't get out of here. I'm gonna hit you in five seconds!
Dean: Five! Four! Get out! Get out! Three... Two!
Dr. Feinberg: Think about your wife, will you? Think...
When Dean and Cindy are embracing outside the clinic in the flashback, a man in the background walks behind a sign and does not emerge until some time later. It could be that the Production Assistant told him to stay there, because you also see the PA step out from behind the tree next to the sign and immediately go back behind the tree again.
The initial credits, showing major cast and crew, play over a montage of stills from the film and clips of fireworks.
