After a young man's premonition of a deadly race-car crash helps saves the lives of his peers, Death sets out to collect those who evaded their end.
(at around 40 mins) Nick Zano ad-libbed the "donkey punch" line in the sex scene. Zano stated in an interview that David R. Ellis encouraged him to make up outrageous lines in various takes.

Janet: No. No, I can't keep doing this, okay? You're both nuts.
Lori: Janet, we gotta go. Come on.
Janet: No. Don't you see? This is where I was supposed to be in the first place, not that stupid race. I was meant to see this ...
(at around 39 mins) Nick says he can't be sure who is next to die, Hunt or Janet, because in his premonition, they were both killed in the same collapse. However, he is confident that George dies before Lori even though in his premonition, they were both killed by the same explosion.
Opening credits run over a "greatest hits" of the kills in earlier installments, presented as 3D CGI X-rays.
Available in 2D and 3D on both DVD and Blu-ray.
