When Rambo stands at Carmen's grave, he places his Medal of Honor on her "tombstone." This has no motivation as far as his character goes. The point he would have gotten rid of that medal would have been in the first film, even the second. (He actually did get rid of all of his medals in the book for the second movie.) If he buried a former Veteran, even that would make more sense. But placing it on a teenage girl's grave is hitting a fake hero moment.
There are no opening credits other than the production company logos and the title of the film. In fact, the title of the film appears immediately after the production logos.
Two different versions have been released theatrically. One version is 89 minutes long and has been released in the US and the UK. The other version, released in various countries, is 100 minutes long. The 100 minutes version has a longer opening sequence where Rambo is helping to rescue people caught in a storm.
