(at around 56 mins) During the "Steam Maze", the trajectory of the trap (a spear that will pierce Debbie's head if she does not locate the key in time) is completely off. When it fires, it is shown entering her neck, but a couple seconds later it is seen sticking out of her skull above her left eye. It should either have gone through her neck and out the back, or sticking out the back of her head instead.
SPOILER: In the Unrated Edition, there's an extra scene after the end credits: Amanda comes to the door of the place holding Corbett Denlon (Jeff Denlon's daughter, who Hoffman "saves" at the beginning of "Saw V" (2008)) and warns her "not to trust the one who saves her".
Despite the R-rated theatrical cut only being available on DVD and in 4:3 "full screen" format, online streaming platform Vudu has this cut in its original 1.85 widescreen ratio and in high definition. Aside from this, rental-exclusive DVDs of the film (such as from Redbox) featured a widescreen transfer of the R-rated theatrical cut, albeit in standard definition.
