Richard would likely not have been able to save his wife even with the medicine from the doctor. Isabella's symptoms are consistent with those of tuberculosis (TB), which had no known cure/immunization until 1906. In fact, the antibiotic treatment for TB would not be discovered until 1946.
I've known him since I was twelve years old, that should count for something.
Frank Lapidus: So you guys met when you were kids, huh?
Ben Linus: No Frank, I was a kid. Richard looked just like he does today.
Frank Lapidus: You're saying this guy doesn't age?
Ben Linus: That's exactly what I'm saying.
The white rock the man-in-black is carrying in the final scene is not the same white rock that Richard Alpert put into the man-in-black's hand earlier. Near the end of the show, Jacob told the man-in-black, "I see you got my present."
