When part of Gotham is turned into a private reserve for criminals known as Arkham City, all hell is sure to break loose, and the Dark Knight is the only one who can stop it.
An Easter egg/reference relating to The Scarecrow can be found on a barge in the docks area. When on the barge, use the sequencer gadget to solve the code "City of Terror". A panel opens. Once inside, you are in a room filled with neon bug zappers. At the end of the hall, there is an unconscious inmate sitting in a chair. Upon zooming in on him, he is startled awake and gives a loud shriek. He goes back to sleep. It is assumed that he is a victim of The Scarecrow's experiments.
Extraordinary, isn't he?
Captain: You knew! You knew we couldn't take him down, you knew we were no match for him!
Hugo Strange: But soon we will be, this was merely the final twist of the knife.
After infecting Batman with his poisonous blood, Joker says he had also shipped some all over Gotham, possibly infecting even more people. By the game's end, Batman manages to cure himself, but it is never mentioned if the other people in Gotham managed to be cured as well. It is implied, however that the cure likely will be sent out to the other infected by way of mass producing the cure which is now within Batman's bloodstream.
SPOILER: At the end of the third Catwoman mission, she is given the option to rescue Batman or leave Arkham City. If she chooses to leave the city, the credits will roll and the voice of Oracle can be heard pleading for help because everybody else has been killed.
