Sex & Nudity

  • Gwynevere wears a dress that exposes moderate cleavage.
  • Three of the bosses in the game, which are the Asylum Demon, The Stray Demon, and the Demon Firesage all have their buttocks exposed and aren't seen wearing anything.
  • Chaos Witch Quelaag, a boss in the game, has a lower body of a demonic spider and upper half is human. Her upper half is mostly covered by her hair but some cleavage can be seen. This all plays out in a cutscene that almost entirely focuses on her chest before the boss fight but during the actual fight you can only notice that she is shirtless.
  • It is also possible to visit Quelaag's sister, who is also a spider with a young woman's upper body. She also has long hair the covers breasts but has cleavage.

Violence & Gore

  • Lots of blood. When the creatures die, though, their bodies are left intact so not much "gore" per se. You can cut off the tails of creatures and there are showers of blood and the bloody stump where the tail was severed remains visible. Bosses disappear after killing them.
  • The blood doesn't look very realistic, though and doesn't stain the environment.
  • You are able to turn off the blood in the game.

Profanity

  • Very infrequent uses of mild profanity such as "hell" and "damn".

Alcohol, Drugs & Smoking

  • The player character can take an item called the Green Blossom which makes you hyperactive.

Frightening & Intense Scenes

  • Many of the bosses you face are very creepy and disturbing in appearance, such as Pinwheel, Gravelord Nito, the gaping dragon, the four kings and more.
  • This game is known as being hard; it is counted among some of the hardest games ever made, but it is also very rewarding; when you finally beat a boss you've been fighting for two hours, you feel a unimaginable sense of accomplishment. The enemies and bosses are all very creepy and may freak out people of all ages.
  • The scariest part of this entire game is that anyone can kill you. You can defeat a boss, then turn your back and get killed by a grunt. You are almost never safe in this game; anything can and will kill you if you aren't careful. The enemies are almost all quite creepy and the setting is very morbid; everyone is dead, save for a few people who can't die, including the player character.
  • You will die hundreds of times throughout this game, so get used to it; if you die, don't cry. This game is based on skill and patience, so giving up will just make the game harder.
  • The game's successor is rated T, and so could this one, really. The blood doesn't look realistic and the violence is all in a fantasy setting.
