Ottaviano Dell'Acqua's hair and beard are visibly longer in the scenes shot after Lucio Fulci left, such as when he is fighting the bald zombie at the abandoned kiosk, and his final scene with the anti-contamination squad.
15 minutes into the film, a room service boy enters the infected mans bedroom to bring him water. After he leaves, he asks a female colleague to clean the room. When she enters, it is clearly a completely different room.
Patricia's leg injury means that she spends the majority of the film with a severe limp: however, this conveniently disappears when she has to jump into a canoe and steps over three or four seats to reach the front.
In the opening credits scene, when the helicopter is approaching the compound and is landing, the interior shots show it almost having landed, while the inter cut exterior shot have it much higher up in the air.

Factual errors

Ken and his two army friends carry sidearms on them, this is highly impossible as enlisted men on a weekend pass would not be permitted to carry military side arms off the base with them.

Revealing mistakes

When Bo and Carol take the jeep to search for a doctor, you can see the flat bed of the truck on which the jeep is being carried when there are close ups of the two talking together.

Crew or equipment visible

When the group arrive outside the abandoned military hospital, a crew member is visible to the left behind a house turning on a smoke machine.
When Commander Bryant is explaining to Professor Holder that not even the military were aware of the potency of Death One (after Holder's line, "You should have told us about the risks involved!") the shadow of a camera is visible on the wall behind the General.
When two of the characters are driving around the supposed to be deserted town, an extra can be seen watching from a window of a house.

Errors in geography

The film is set in California but the geography is obviously the Philippines.

Plot holes

Dr. Holder states that there's a radioactive cloud in the air, yet a few seconds earlier, a scientist comments that Death One isn't radioactive.
