(at around 1h 21 mins) The camera is reflected on the scythe after it cuts off Chucky's face.

Continuity

(at around 38 mins) Chucky cuts Andy's Achilles tendon from under his bed. Throughout the rest of the movie, Andy should have a limp from such a severe injury. However, he clearly has no trouble walking or running in subsequent scenes, nor does he show any signs of injury.
(at around 1h 22 mins) The position of Andy's hands changes between shots as he hangs from the rollercoaster's track and the car passes over him.
(at around 39 mins) When Andy is fighting with Chucky in his room, he throws a shoe at Chucky, causing him to drop his pocketknife, which lands under the bed. In the next shot, the knife is beside the chest of drawers.
(at around 20 mins) When Andy removes the white rag used to gag Whitehurst, it is still wrapped around his neck, but in the next shot, it is lying on the floor. When Whitehurst stands up, it is back around his neck.
(at around 54 mins) When Andy falls to the floor with his tray, a lot of food falls on the floor. When it cuts to a wide shot, all the food is gone from the floor. Additionally, the tray has moved closer to Andy.

Factual errors

Female cadets are required to tie their hair back while in uniform. Also, despite Botnick being so hard-nosed about hair, even he allows male cadets to grow their hair longer than it should be, and doesn't cut Andy's hair short enough at the beginning per military standards.
(at around 1h 21 mins) No amusement park ride company would ever produce a prop scythe that was as sharp as depicted in the movie.
While it is common to hear female ranking military officers referred to as "sir" in movies, this is not done in the military. Female officers are referred to as "Ma'am," or by their rank or title. For some reason, Hollywood persists in maintaining this odd bit of folklore, but there is no truth to it.
Tyler plays video games while in military school. In reality, cadets would not be allowed to play games while in military school and, as such, Tyler should have had his console confiscated.
The deaths of the garbage man, Sergeant Cochrane, and the military school barber should have had the police involved to figure out how they died - though it is possible the police were involved off-screen.

Incorrectly regarded as goofs

(at around 25 mins) Chucky mails himself as a means of travel, including somehow wrapping his own box from the inside. However, it's possible that he posed as the Play Pals CEO and left a note for his secretary to wrap the Good Guy doll in his office and mail it to the appropriate address, then climbed back inside the box and simply waited to be mailed.
The Play Pals company completely abandons the factory that was producing Good Guys dolls for eight years, letting it become neglected instead of using that space to produce another one of their products as any financially responsible company would do. It is stated in the film that the company went out of business and is only beginning to start work on all new products, not just the Good Guy line.
(at around 51 mins) Col. Cochrane thinks nothing of finding Chucky - the same doll he had thrown in the dumpster earlier that day - back in the school, after De Silva had put lipstick on him. Does he not remember throwing him away? He could think it's a joke/prank a cadet or someone else is playing on him. Also, he wouldn't know it was the same doll.
(at around 38 mins) Chucky somehow recognizes Andy; given the movie takes place eight years after the conclusion of Child's Play 2 (1990), Chucky should not be able to tell who Andy is as his appearance and voice would have changed. However, Andy's room has his name on the door, he is new and introducing himself to multiple people right and left, and Tyler could have easily told Chucky where to find Andy off-camera. Not to mention, barring some seriously outrageous circumstances, most people can at least vaguely recognize someone who they knew as an eight years old.
Tyler only looks about nine years old, too young to be in military school. It should also be illegal for someone that young to use rifles. While it is illegal to sell, transfer ownership of, or possess a handgun in Illinois, it is not against the law for a minor to operate a long gun, such as a rifle. If this were the case, none of the cadets would be able to operate a gun, leaving the film without a major plot element.

Revealing mistakes

The snow in the forest at the end of the movie is more than likely foam, as it bends and wiggles underhand. And 6 mins from the end, the red flames rising from the circular thing at the amusement park are obviously pieces of fabric cut in pointy shapes. There are so bad and obvious, they are shown several times.
(at around 4 mins) During the Play Pals meeting, slideshow photos of Andy and Charles Lee Ray are shown. All of these photos are publicity shots/still frames from the first Child's Play (1988) film, and not actual photos that should exist in the world of the film.
(at around 15 mins) The newspaper clipping Col. Cochrane reads while speaking to Andy at the beginning of the movie clearly has nothing to do with the story of Andy and Chucky; rather, it is a series of unconnected paragraphs about nothing in particular.

Miscellaneous

When Andy and D'silva are in the gun range, they wear neither ear or eye protection.
The red team fire live rounds instead of pellets. The team should have realized this once they heard the gun shots and ceased fire.
When doing armed drill exercises, all of the students hold their rifle at different angles and positions, which would never be allowed without correction. Beyond this, when they are called to Right Shoulder Arms, nobody salutes the rifle (open palm position with forearm perpendicular to the rifle and fingertips touching the bolt) which is part of that movement.
Chucky should think he would be hurting his chance to possess Tyler when he scares Colonel Cochrane and gives him a heart attack. There is nothing he could likely do to Cochrane given how far away he is. So Chucky doing this doesn't make any sense.

Crew or equipment visible

(at around 34 mins) When the garbage man hears Chucky's cries for help, the boom and camera are visible.
(at around 12 mins) The wire attached to the toy helicopter making it "fly" around the room is visible.
(at around 1h 21 mins) As Chucky walks towards the camera in the rollercoaster scene, the shadow of the mechanism moving his feet is visible.

Plot holes

(at around 12 mins) Chucky activates several toys around the Play Pals CEO's apartment to distract him, including two other Good Guy dolls. Chucky is the first Good Guy to be made since the toy's discontinuation eight years earlier, so these other dolls should not exist unless, for some reason, they've just been sitting there for eight years.
The movie opens with people clearing away doll parts that had been there for years and Chucky's remains are carried away causing his blood to fall into a vat of molten plastic which forms a new doll. The doll is presented as the first doll off the assembly line. This makes no sense. The place should have already been cleaned if they were making new dolls.
(at around 54 mins) During the breakfast scene, Sgt. Botnick tells Andy he needs a trim on Wednesday, but he had just given him a haircut the previous day.

Boom mic visible

(at around 1 mins) The shadow of the boom mic is visible on the vat of plastic at the beginning of the movie.
(at around 19 mins) The shadow of the boom mic pole is visible moving to the right of the door when Andy gets to his room for the first time.

Character error

(at around 43 mins) When Col. Cochrane emerges during the midnight exercise, he is not wearing his cover (hat) as he should be. He also salutes improperly (his hand position is wrong) and inappropriately, since saluting is never done without wearing cover. Saluting is also never done indoors (since soldiers or cadets never wear cover indoors), despite this being seen several times in the movie.
(at around 14 mins) When Chucky uses the CEO's computer to look up information on Andy, the word "juvenile" is misspelled as "juvinile".
(at around 24 mins) In the post office, Tyler salutes to one of the officers, but he should only do so if he wears a hat as cadets cannot salute without wearing a hat.
(at around 43 mins) Col. Cochrane tells Shelton that he wants everybody back inside from the midnight exercise at 0100 hours (which is 1 a.m.). However, he pronounces it wrong by saying "Oh-100" hours (as in, the letter "o" from the alphabet). Proper military pronunciation would be to say "Zero-100" hours.
Shelton orders Andy Barclay to fall out during the drill exercise after Barclay struggles with the rifle. However, Shelton fails to notice DeSilva mishandling her rifle right in front of his face. DeSilva repeatedly moves her head around the rifle as she changes positions with it. This is incorrect; she should be moving her rifle around her head, and keeping her head still. As hard-nosed as Shelton is, there is no way he wouldn't have noticed that, especially with DeSilva standing right in front of him.
