Incorrectly regarded as goofs
Rick Ducommun is credited as Ripper's agent. He wasn't Ripper's agent. He was Tom Noonan's agent. Clearly Noonan was best known to the public for playing Ripper, so it wouldn't be unthinkable for people to say this.

Continuity

When Benedict fires blindly at the target next to Frank and the camera zooms in towards him, his revolver has a threaded line holding it up steady for the perfect bullseye shot.
When Jack is leaving the police station with his personal effects in a box his shirt is riddled with bullet holes. When he arrives at his apartment and enters, the shirt has no holes.
When Danny is watching the movie and the ticket activates, Jack shoots the the dynamite which flies out of the screen and lands in the movie theatre, Danny runs towards the screen to take cover. Later, after Benedict leaves the movie, Jack and Danny follow him and land in the same movie theatre and there is no sign of the explosion that would have resulted from the dynamite.
After Benedict shoots Jack, he loads two bullets in the gun to shoot him again. Then, when Slater shoots his eye, one shot. But when Danny points at Death, the gun is full.
When Slater is in the elevator and the helicopter hovers outside, the twin machine guns and mini-gun switch sides on the chopper repeatedly.

Factual errors

In the end credits listing for "Terminator 2: Judgment Day," the word is misspelled as "Judgement."
In the "Featured Musicians" credits, Rick Nielsen's last name is misspelled as "Nielson." This is not corrected on the Blu-ray release.
After playing chicken with the driver of the taxi cab, Jack Slater would not be able to walk away from the accident. In fact, he would have some type of injury that would likely require a hospital visit.
The real City Coat of Arms for Los Angeles has the American federal shield (a white field with six red thin vertical stripes on the field - creating the illusion of seven white, and six red stripes, for a total of thirteen stripes, and thirteen white stars in the blue Chief) in the first quarter (upper left quadrant of the field); the California state flag (A five-point star in the upper right corner of the field, a grizzly bear, and a horizontal stripe in the base; all in red) in the second quarter (upper right quadrant of the field); the Mexican eagle atop a green cactus (from the Mexican flag) in the third quarter (lower left quadrant), and the parted arms of alliance for Castile and Leon (representing Spain) in the fourth quarter (lower right quadrant of the field). When Jack Slater pulls into the parking lot at Police Headquarters (actually a Sony Pictures office building), the Los Angeles Coat of Arms out front has the American eagle (with a federal shield with four stars in the Chief in escutcheon) instead of the Mexican eagle in the third quarter, and a black rhinoceros (instead of the red Bear) in the California state flag in the second quarter.

Incorrectly regarded as goofs

Leo La Fart's funeral is in Long Beach. The movie shows the La Brea Tar Pits as being near the hotel. It is many miles away in Los Angeles. This is a deliberate reference to the "creative geography" goofs done regularly in the movies.
Death is not shown to re-enter his movie, yet the fact that the Grim Reaper is on the loose in reality and killing people is never mentioned again. He could have re-entered his movie off camera. Additionally, Death seems to have no ill will towards the living and is only after people who have reached their death date, so he's of no real threat anyhow.

Benedict is able to kill the mechanic in front of his shop, despite the fact he is not close and the bullets should be blanks. However, in the universe of "Jack Slater IV" the bullets are not blanks and are shown killing characters, so presumably the "live" bullets would have transmuted into the real world along with Benedict.

Revealing mistakes

In the projection booth, when the film is out of focus, Nick goes to adjust it while the credits are rolling, yet the reels on both projectors are empty and not moving.
Danny and Slater enter a movie store called "Acme Video", But the door says Blockbuster video and a few moments later, a Blockbuster Video sign is clearly visible on the wall.
When Slater throws the body over the edge at the funeral, he clearly throws it onto the flower bed right behind him.
The rear-view mirror has been removed from the Mercury Sable (the vehicle that Jack steals).
When Jack falls from the broken elevator, the crane is moved right below him so he can grab on to Leo the Fart. However, Leo is not attached to the crane, but when the shot changes, Jack grabs on to Leo the Fart's legs.

Miscellaneous

After Death leaves the screen of The Seventh Seal, he eventually tracks Danny and Jack to the theater after Jack has been shot. As Danny stands to protect Jack, Death says that he has no knowledge of Jack (presumably since Jack is portrayed as a fictional character in the movie). Death does tell Danny that he's familiar with him, but then advises that he will die as a grandfather. Since Death was also portrayed as a fictional character, he should have had no knowledge of Danny, since Danny was portrayed as a real-world character.

Audio/visual unsynchronised

Jack Slater plays a game of chicken while in the real world the explosion is heard before the cars actually crash together.

Crew or equipment visible

After Benedict shoots Vivaldi and he is talking about moving into parallel worlds, crew members are visible in the mirrors.
When the house explodes (Jack's favorite second cousin), the hydraulic jack pops up to flip one of the police cars into the air.
While in the real world (New York) Jack Slater plays a game of chicken with a taxi. When his car starts going down the alley, there clearly is a cable running across the floor attached to Jack Slater's car.
Hands holding colored spotlights can be seen in the theatre's projector room when the magic ticket is used the last time.
When in Benedicts house, as The Red-eye is lingering, a dark glass window reveals the camera man.

Plot holes

Jack Slater walks around New York City without anybody thinking he was Arnold. He most likely would have been mobbed by fans.
When Danny holds up the ambulance at the end of the film, he commits a crime and should have been accused of a car jack and possible attempted murder charges. Yet, the film ends happily without him facing any kind of charges, yet the ambulance workers could easily recognize him, this makes no sense.

Character error

Danny Madigan states that after the prefix 555, there are only 9999 possible phone numbers, since only four digits are available. This is incorrect: there are 10000 possible phone numbers. He forgets that the combination 0000 is also available.
