There are surgeons Dr. Zack Nolan and Dr. Owen Hunter who perform surgery on people and blood is shown as he takes out the bullet wound. Dr. Zack later has an overwhelming task of performing surgery on patients at the hospital and later in tent city.
There are riots in the street as people fight the police and military soldiers to get out of the city.
Dead bodies are shown briefly in near-full graphic detail.

Frightening & Intense Scenes

The opening scene with the stunt biker in Seattle Washington city happens as the earthquake rocks the city and the stunt biker is seen riding his bike during an earthquake dodging falling buildings riding over cracked roads, a man gets out of his car and runs as the telephone pole falls on his car, the biker looks up at the Seattle Tower which cracks and the biker races to escape it only to have lean over and fall on him, it's unknown if the biker survived the building crash or not.
There is a scene where the second earthquake crack causes the railroad tracks to fall into the cracks and causes the train to crash in the crack. It's unknown if the people in the train have survived or not or if they have been rescued.
The earthquake sequences can get very intense.
