There is blood when bailing but it's very mild and is unrealistic. Sometimes when bailing, your skater will say "ah my ankle" or "my knee" and they will clutch them.

Like the other games in the series, there are some unlockable bails videos which shows skaters injuring themselves while trying to perform tricks. One part in the end credits shows the aftermath of a Neversoft developer who knocked some of his teeth out while skating. Some blood is shown in a sink and in his mouth.

Occasionally people swear. Mike V also uses the word "bullshit" once.

Some people are shown to be drunk throughout the game and waking up with hangovers as well.

In the New Jersey level, there are a group of "dealers" who are implied to be drug addicts. One of them has weed leaves on his shirt. Others are shown to be tweaking while standing still implying they are high on something.
