When Sam Dexter cuts the brake line on Vicki's car the line is obviously made of soft plastic. This kind of line would never stand the pressure found in a car's brake system. Brake lines are often reinforced with steel mesh to withstand the pressure. Also the "brake fluid" is shown as a red liquid like automatic transmission fluid. Brake fluid is clear or slightly yellowish.
A car wouldn't burst into flames just from hitting a guardrail. It often won't even burst into flames when crashing to the ground below.
When Barney & Sam are discussing escaping while at the pool table, the position of the balls keeps changing, as Barney and Sam standing at the far end of the table, there is a pale blue ball next to the cue ball directly in front of them, the next time, before Barney takes the shot the blue ball is gone and the cue ball is now along the right side of the table.
During the fight in the pool table scene, Barney is clearly replaced by his stunt double.
While "Murdoch" is in the hospital bed on a cardiac monitor, the white and black electrodes are reversed. The white electrode should be placed on the patient's upper right chest and the black on the upper left, not the other way around.
Barbara Rhoades's character is referred to as Vicki Wells during the episode, but in the end credits she's called Vicki Holt.
A cable can be seen attached to Vicki Wells' car as it goes over the cliff. Also, flames can be seen under the car before it even impacts the guard rail.
The IMF would have no idea how henchman Ben Murdoch is dressed. However, the agent who replaces him (via mask) is dressed exactly the same, right down to Murdoch's striped shirt.
(At 0:29:00), the shadow of the boom mike is visible and moves along the right and top hospital room wall. The studio above the set walls is also visible.
When Ben Murdock goes to visit Sam Dexter in prison, he mistakenly calls him "Ben".
