Like its predecessor, Lust From Beyond is an erotic horror and has strong depictions of sexuality and sexual violence. The sequel has an even darker and more graphic tone than the original, and this time allows the player to actively engage in some sexual acts via interaction. Rape and sexual violence is often depicted throughout the story, though the player's interactive moments are generally consensual.

After one dream (which includes a moment of interactive intercourse), the main character wakes up and notices semen on his hand.

Masked orgies take place in both cults, with the ability for the player to engage in interactive sex (e.g. Receiving fellatio, or giving rear penetration). In one of the cults these are optional and consensual, while in the other one Victor is required to undergo them as a captive.

Later in the game, the player acquires a knife and a gun. During combat the enemies can be sliced or shot at with significant blood squirt, but there is no gore or decapitations.

The gore that is included, is depicted within the game's environments e.g. When visiting the Scarlet Lodge's chambers, there's many dead bodies of torture victims and human remains scattered throughout.

A naked corpse is hanging above a dining table, with the stomach sliced open and the entrails dangling down. Very disgusting.

One of the key Scarlet Lodge members, Timothy is often seen carrying a severed leg (which he also uses to attack Victor).

Lusst'ghaa is a nightmarishly bizarre and frightening world. It has a Giger-like aesthetic, with many abhorrent references and depictions of sex and lust. A lot of frightening creatures are also found throughout. Many of the game's moments take place within there.

The Scarlet Lodge is a highly sinister and unhinged secret society that indulges in extreme violence and grotesque rituals. Its members are deranged sadists that take pleasure in disgusting sadistic practices, which include but are not limited to: rape, torture, necrophilia, sadomasochism, coprophilia, and even implications of cannibalism. The main character witnesses each of these happening at some point (or the aftermath), and at times gets beaten and tortured himself.

In one sequence, Victor is pursued by Scarlet Lodge cultists across some rooftops. The chase is intense and during it, the player character glimpses many dead (nude) victims around the place, and intimidating bloody writings on walls like "WE ARE YOUR PAIN"

One particularly disturbing chapter has the player character explore a large underground chamber which is drenched with naked corpses hanging all around the place: some of them are in cages, a few in iron maidens, and others are held by rope with their bodies in overstretched positions.
