Pretty violent, more then the show is normally and even more violent then the THOH specials, but not violent as Family guy and Rick and Morty.
Flanders and Homer fall off a rocky hill and crash into a fallen log. Probably the least violent scene in the episode since is more in the vain of slapstick rather than realistic violence.
Kostas' goons staple Comic Book Guy's ponytail into the wall and destroy some of the collectables found in his store.

Homer said Crap 7 times while falling down the hill.

Homer goes on a drunken rant towards Flanders after he donated the money they found on the log at the beginning of the episode.
Afterwards, Homer is thrown into the "Drunk Rink", in which other intoxicated citizens are seen. Although, this is played for jokes as Homer and the other citizens over drank eggnog, not alcohol, thus not actually drunk.

The tone of this episode is alot darker than the usual Simpsons or Treehouse of Horror episode. There's still some comedy sprinkled around it but there is usually a feeling of tension, suspense and shock present that it outweights the lighter moments.
The violence in the episode can be pretty grim and disturbing to younger and sensible viewers.
Overall, this is one of the more mature episodes of The Simpsons, rather than dealing with more real topics, featuring more jokes and sequences, or parodying something, the tone of the episode is more aligned for a more older audience, or younger viewers that are mature enough to understand the violence. A pretty deserving episode of it's TV-14 (DLSV) rating.
