At one point, you obtain a sledgehammer and have to fight humans and aliens alike with it, resulting in a lot of blood and gore, and the hammer potentially being caked in blood and brain matter.

This title has a focus on fighting, shooting, and killing others. Players will use a variety of weapons to accomplish this. such as, knives, bayonettes, sledgehammers, grenades, pistols, assault rifles, machine guns, sniper rifles, shotguns, and a handful of alien weaponry.

There is a lot of graphic gore and violence in this game. Chimeran's die in a large spray of blood each time you kill them. Certain weapons can literally make an enemy explode into many pieces, while others can literally cut an enemy in half. Lots of brutal and graphic violence in both the main gameplay and the cutscenes. There is a very large amount of graphic and brutal gory violence.

A few uses of the F-word in the campaign, though they're pretty rare, and there's not a whole lot of swearing throughout.

Plenty of points where you navigate ruined towns and cities, and the atmosphere of the former is rather creepy because most enemies are zombie-like foes and ammo becomes limited.

The first two games could be intense because of their brutal battle scenes, whereas this one plays more like a survival horror at plenty of points with you surrounded by disturbing, sometimes zombie-like enemies with no support and even limited ammo.

The appearance of the Chimerans is very disturbing and frightening. They have four eyes, deformed mouths, decomposed skin almost and carry alien weapons around with them, with strange life packs, it seems, on their backs.

The violence in this game is horrendously graphic and sometimes in your face, which can scare and frighten some.

The appearance of some of the bosses are frightening and disturbing, like the Widowmaker's.

The Warden's in the game are savage, brutal survivalists who have no qualms with killing anyone who get's in their way. These enemies are very brutal and disturbing.
