Some sexual references are in the game
A woman mentions that she and her boyfriend would "make love" in the boyfriend's car.
A woman's bare breasts can be seen while she's taking a bath. During this scene, another woman wants to come into the bathroom to ask the previous woman mentioned a question. Depending on dialogue choice, the woman in the bath can either say she's naked and that the question can wait, or the other woman can come in, if she's not a lesbian. The woman then comments that she doesn't want a lesbian to stare at her boobs. The second woman says "If I want to stare at some boobs, I'll take my top off and look in the mirror". Scene is meant to be funny.
A naked, morbidly obese woman is seen at one point in the game.
A woman asks another woman if an intruder is going to rape both of them.
A man argues to his wife that she never kisses him, or even has sex with him, anymore.

Lots of grotesque imagery, considering the video game is a psychological horror all about depression.
In the second chapter, a woman falls from a tall building, to her death. Later, a man stabs a woman in the stomach. In the second "dream" sequence, there are various scenarios of dead women portraying famous paintings, such as the Mona Lisa. The same man is later shown wearing a mask while torturing a woman who keeps blood-curdling screaming. The protagonist then kills the man with either a spear, or a mace.
In the third chapter, there is nothing scary. The protagonist is seen returning home, with blood all over her.
In the fourth chapter, the protagonist is kidnapped by a couple, who happen to be cannibals. This chapter shows lots of creepy things, especially the wife. At the beginning, a woman pours bleach over the protagonist, blinding her. In the chapter, the wife can be seen chopping bloody raw meat, assuming the meat is human. At one point, you see a decaying corpse in a bathtub full of green water. The protagonist poisons the husband, then she shoots the wife with a shotgun.
In the fifth chapter, the protagonist and another woman are invaded by an intruder, who beats the protagonist to death with a hammer. The protagonist has another "dream" sequence. This time, she's trapped in her apartment that's literally covered in skulls. Later, an aged version of the protagonist gets slaughtered with a saw-blade (with blood splattering everywhere). Then, the protagonist stabs a scary-looking creature to death. After the sequence, the two characters are shown completely wrapped in duct tape, in the bathtub. Later on in a different scenario of the chapter, the protagonist reveals that she had a baby daughter, who died due to a rare allergy. She also had a husband who drank himself to death. Afterwards, the secondary character is seen about to be hanged. The protagonist summons a group of cats, and the cats literally eat the intruder alive.
In the sixth chapter, however later on, the protagonist walks into someone's apartment, and encounters a door that has the number 666 on it. She later falls into a hole, thus leading to another sequence. In this sequence, the protagonist encounters a woman, who shrieks in a demonic voice "MISERY!". Later, the protagonist readjusts several mirrors, that reveal the mysterious woman's rather demonic face, cackling over and over. Then, depending on the player's choice, the protagonist can either strangle the creature, or be shot. After the sequence, and later in the chapter, the protagonist tells a scary story, while the scene changes in the act of the protagonist pranking one of her neighbors. At the end of the chapter, a man is shown with a morbidly obese woman, who is seen shaking erratically, and screaming in agony.
In the final chapter, the protagonist and secondary character meet up with the final enemy (who is paralyzed) of the game. In this chapter, the enemy poisons his father, and (depending on the player's choice) poisons either the protagonist or the secondary character. After that, depending on the player's choice, the protagonist or the secondary character makes the apartment room explode, thus killing the secondary character and the enemy in the process, or let the enemy live, and have the two characters walk away.
The secondary character mentions that her boyfriend committed suicide.

Lots of frightening scenes (see Violence & Gore)
There's some dialogue mentioning harm to cats, which can upset people who love cats, but nothing happens.
