Director John Ashcroft said the film is ultimately about tyranny and described the story as the rise of a dictator in the least of likely places.
To add authenticity to the film, all of the actors in roles as caretakers received training from an actual care facility; like how talk to patients, how to pick pick them up using a harness, and how to walk with them.
