There is discussion of casual sex, drunken sex, infidelity, and unplanned pregnancy in many portions of the game. However, none of it is endorsed and portrayed in a disturbing/depressing manner.

The ghost in the Ermengarde mansion is a bloody, gory abomination with flesh and bone exposed and blood seeping all over her face.
The Letter itself is written entirely in blood.

The Letter is extremely intense and graphic in places, with many bizarre and horrifying deaths supposedly caused by a ghost.
When your character encounters the ghost, you will be forced to play a quick-time-event to avoid its attack. If you fail the quick-time-event, it results in a jumpscare, which is almost always a close-up of the ghost's hideous, zombie-like face.
Many of the deaths in the game are sudden and appear out of nowhere, often without any warning, meaning anyone can die at any moment.
