Also in the Thalamus area, several of the scrawny people suffering under Thalamus's power can be found, and you have the option to kill them all. One of them can be seen violently bashing his head against a stone pillar, little splashes of blood are seen.
The main form of gore in this game are the piles of bodies in the background. Some of them are fully intact with blood gushing from their mouths and noses, some are decomposing, some are gored up with organs and ribcages exposed, etc.
While fighting the Avatar of Thalamus(the final boss of Ending A)he splits in half from the top of his head down to his crotch after you hit him a couple times, and there's lots of blood. He quickly heals himself and keeps fighting, though, so it's very brief.
A large statue/fountain is seen a short time before the Origa boss fight. It is of a man cutting deep into his arm with a dagger, releasing red liquid obviously representing blood into a chalace held by a statue of a man kneeling. It's just a fountain, yes, but it's still pretty gross.
If you die during the Origa boss fight, she will lock you in the dungeon that's for some reason in the basement of the church that you fight her in. In order to escape, you have to kill yourself with the suicide dagger. Nothing graphic is shown, you just use the item and then Sorun collapses, but it's still kind of dark and it's very much a representation of suicide.
A couple representations of suicide, some pretty disturbing. In the prison area, a man inside of a cell begs you to kill him, and then proceeds to tell you that he'd do it himself if his hands weren't shackled.
A large portion of the game is all about hacking through enemies with various weapons, including axes, swords, daggers, scythes, spears, etc. Some enemies will just collapse and be dead, while some will get chopped to pieces upon death. And yeah, the severed heads roll.
An armored character can be seen in the main hub area of the game, kneeling over their dead comrade. There's definitely enough blood on the corpse and the hay bail its propped up on to be gross, but not enough to be called straight up gory.
The prison area is also full of scrawny, near dead people who have already succumbed to the will of Thalamus, an eldritch god that forces his victims to relive their greatest regrets for eternity. Talking to them will give you one of a couple voice lines, such as, "Just let me die!" or "Thalamus..." or, darkest of all in my opinion, "I repent! I repent!"
A common thing seen in the background is a body impaled on a spear. Blood can be faintly seen dripping from the tip of the spear, although the blood drops are so small it can only be seen on a large screen in docked mode. There are also(seemingly still alive)humans stripped down to tiny loincloths with large gashes across their chests, hanging from trees by their arms in the snow area. In the prison area, bodies can be seen impaled brutally on stalactites, their faces frozen with horror.

You can purchase beer from a little purple lizard, but drinking it harms you so I guess that discourages alcohol use. Take that how you will, though.

The game has a very serious and very mature theme of how death is a part of life and the way people cope with it. The game also deals with heavy and pretty dark topics like eternal punishment and stuff, which can freak some kids out.
The main antagonist, Thalamus, is a pretty frightening character, especially for younger kids. He is an Eldritch being that feeds off of people's trauma by forcing them to relive their greatest regrets for all of eternity. Many people can be seen under his influence in the game, and they are clearly suffering badly and begging for mercy. Every time you fight him(except maybe the third time)there is a strong sense of hopelessness and chaos throughout.
There is a scene where soldiers come to take a child away from his mother. It's a pretty upsetting scene, especially for little kids.
